Job Description


Medical Record Coordinator

Under the supervision of the Regulatory Risk and Health Information Manager, the Medical Records Coordinator will be responsible for but not limited to retrieving documents from various sources (Epic, Hospital, Nursing Homes, Doctor’s office, etc.) and attaching documentation to participants chart, and responding to request for medical records from participants/caregivers.

Medical records coordinators are in charge of managing medical records and coordinating related activities for PACE Southeast Michigan. They are responsible for keeping medical records secure and they must respect privacy concerns for all participants. They will perform confidential administrative functions to ensure assigned departmental operations work correctly and that only the necessary parties have access to the information they require.

SPECIFIC DUTIES AND FUNCTIONS:

  • Retrieves medical records from various sources, external and internal this is to include but not limited to:
    • Medical
    • Surgical
    • Specialist Services
    • Lab
    • Inpatient
    • Nursing Home
    • Emergency Services
    • X-Rays
  • Respond to medical records request via subpoena, attorneys, nursing home, (CMS, state of MI, for auditing purposes) as well as participants and caregivers request for medical records regardless to site.
  • Perform monthly/quarterly record maintenance for all new enrollees, emergency, hospital, nursing home and specialty service to ensure participant chart is cohesive and accurate.
  • Upload Level of Care Determination documents to the State of MI CHAMPS web site report/communicate to the organization monthly/quarterly of LOCD’s due/outstanding.
  • Audit and discontinue all open/expired authorizations/orders, alerts etc.,
  • Assist in the training of incoming staff members

KNOWLEDGE, SKILLS AND ABILITIES:

  • Bachelors preferred in Health Information Technology
  • Working knowledge of Medicare Advantage program, HIPAA, FWA, ICD-10 and CPT
  • Must be flexible and able to handle large case volumes (150-350 participants) with the ability to travel to various sites as needed
  • 1-2 years prior work experience in the medical fields preferably medical records
  • Working knowledge of State of MI DPOA/POA laws and medical records release laws in death and life and retention, and fees.
  • Working knowledge of medical terminology and electronic medical record system
  • Demonstrated skills in MS Office (Excel, Word, Outlook and Power Point)
  • Ability to manage significant work load and to work efficiently under pressure meeting established deadlines with minimal supervision
  • Ability to maintain confidentiality, flexibility, dependability and professionalism.
  • Ability to work flexible day shift schedule (some Saturdays required)
  • Demonstrated effective organizational and planning skills.
