The Sr. Value Based Care Project Manager is responsible for managing major projects and multiple mid/small size projects to enhance patient care and outcomes in a value-based care environment. This position will serve as the organizer, facilitator, planner, problem solver and coordinator of resources to manage all aspects of a project, with direct accountability for project execution. This position will engage key stakeholders in the development of recommendations based on collective input, observations, data and analysis while providing overall leadership to the project team(s) to accomplish project objectives within a given period of time. Strong understanding of value-based care, operations, and clinical or technical expertise is required to design and drive projects to sponsor satisfaction.
This position will focus on the support and implementation of remote patient monitoring, the integration of technology within Epic to support value based care strategies, and leveraging risk methodologies to understand patient populations. This position supports and partners closely with IT and operations across ambulatory, inpatient, and the health plan.
MINIMUM QUALIFICATIONS:
Education, Experience or Equivalent Combination:
Bachelor's degree in healthcare, business administration, or related discipline.
Requires 7 or more years of experience in the health care industry
5 years of demonstrated project management experience
Education requirements may be substituted with related work experience

PREFERRED QUALIFICATIONS:

Master's degree in health care, business, or related field.

Project management or process improvement certification

Process improvement training and implementation experience in a healthcare setting
Understanding of value-based care models, quality improvement, and reimbursement structures
Understanding foundational value-based care strategies and tactics
Return on investment evaluation experience
ESSENTIAL DUTIES:
- (65%) Project Management - Leading, Executing, and Evaluating
- Leads complex and strategic value-based care initiatives/projects across the HealthPartners' Enterprise focusing on major projects and multiple mid/small size projects.
- Develops, coordinates, facilitates, organizes and oversees project plans for value based care initiatives; remediating issues, identifying and coordinating resource needs, identifying risks and ensuring projects meet timeline and budget requirements.
- Partners with executive leaders to scope work, establish performance goals, provide project status, present results, and remove barriers
- Partners with senior leaders, management, physicians, and front line teams to understand business needs, create recommendations based on input, observations, and data and analysis, and implement change across Healthpartners' service areas.
- Analyzes operational systems and data and analytics to identify improvement opportunities.
- Drives to and delivers high quality solutions to stakeholder satisfaction
- Fosters communication, cooperation, and builds consensus between key stakeholders and all people involved in a project
- Partners with leaders and project teams to ensure appropriate standards, technologies, training, and measurements are developed and implemented for monitoring project success and long-term sustainability
- (20%) Coordination of Value Based Care Projects Across the System
- Consults, makes connections and coordinates project work and initiatives across the HealthPartners' Enterprise
- Consults and/or advises care group operational and technical business partners and project teams regarding value-based care initiatives to inform on deliverables, interdependencies, and data insights to ensure collaboration and connection across and between service lines, departments, and project teams.
- Ensures value-based care project requirements and best practices are integrated into project planning for entities and systems across the HealthPartners' Enterprise
- Monitors and reports on progress of multiple projects across the system to ensure milestones and timelines are being met
- Acts as a change agent in development, planning, implementation and measurement of value-based care strategies and initiatives.
- (10%) Value Based Care Team Strategic System Collaboration
- Participates in system wide collaboration with interdisciplinary teams across the HealthPartners' enterprise including, VBC core team, contracting, data ops and health informatics, health plan, finance, Epic, and others to maintain knowledge of the value-based care model, opportunities for improvement, and industry best practices
- Shares and incorporates value-based care best practices and strategies into project work plans, goals and deliverables.

- Participates in various committees, task forces, projects and quality improvement work, as needed and assigned.
